Hamilton Health Sciences MS Clinic Patient Survey

Thank you for participating in our survey. Your responses will help the MS Clinic improve the services it can offer you and other patients.

Please refer to the ID Code in your letter (it is also called a "token"), you will need it to begin the survey.

To begin, please press the "Begin Survey" button below.  It will take about 15 minutes and remember, when you complete the survey, you will automatically be entered into the draw for the $250 prizes.  

Please note that we have sent out reminder letters.  If you received a second letter but have already completed your survey, don't worry, your responses were recorded and you have been entered into the draw.  (You cannot complete the survey more than once.)  If you have any trouble starting the survey, please contact us directly at McDSL@McMaster.ca
